About the Role:
The purpose of the Primary Care Coordinator is ensure the development and maintenance of partnerships between the Capecare residential aged care homes (RACH), GPs who provide care and services to our residents, and other relevant community health care professionals.
Based in the Capecares Dunsborough facility, this role will see you facilitating optimal communication between all elements of the health sector, especially those channels involving GPs and the RACH.
You will help drive an improvement in the quality of care, services and outcomes for Capecare residential clients through enhanced integration of all primary care providers involved in services.
Your key responsibilities will include:
Develop strong collaborative relationships with relevant service providers such as GP’s, Allied Health and Pharmacists to create a partnership in care approach.
As a core part of the Capecare clinical team collaborate with GP’s to facilitate efficient, timely and responsive primary care for our residents.
Facilitate communication between GP’s, Capecare residential staff, and residents, as well as family/carers, ensuring all parties are informed and involved in the care process.
Facilitate communication between other areas of the healthcare sector and Capecare.
Support knowledge and skill development among other clinical and care staff.
In consultation with the Quality & Clinical Governance Manager:
Develop clinical protocols with GP’s to guide clinical decision making, triage, assessment and clinical escalation
Maintain involvement and oversight of GP’s providing treatment to residents
Develop and manage on an ongoing basis an Implementation Working Group in line with the GPAIC guidelines which consists of Capecare senior clinical leaders, GP’s and the WAPHA’s Activity Lead.
About You:
You are an experienced Registered Nurse, looking for a change of scenery. You are a strong relationship builder and will use these skills to build strong partnerships with General Practitioners and healthcare providers to support high-quality care for our residents.
A confident communicator and natural networker, you enjoy being out in the community, strengthening professional relationships and representing Capecare.
You understand the importance of coordinated care and are passionate about improving communication between providers to enhance healthcare quality, accessibility, and continuity for residents. Combining clinical insight with strong stakeholder engagement, you proactively foster GP involvement and identify opportunities to grow trusted partnerships.
To be successful in this role, you’ll need:
Bachelor of Nursing – AHPRA registration
Postgraduate qualifications in dementia care, palliative care, primary care or mental health nursing (desirable)
Minimum of 5 years’ experience in Aged Care
Experience in dementia and palliative care
Demonstrated knowledge of the Age Care Act and Standards
