Are you an experienced care worker - or a nurse no longer registered with APHRA looking for a meaningful change? Join our Transitional Aged Care Program (TACP) and help older Australians recover and regain their independence after a hospital stay.
The TACP is a short-term, rehabilitation-focused program supporting older people as they transition from hospital back to independent living at home. You'll play a key role in helping clients achieve their recovery goals during this critical stage.
What's on Offer
- Level 3 rate: $46.91 per hour
- Paid travel time and kilometres
- Monday to Friday roster, between 7am and 5pm (must be available 2-3 days per week)
- Guaranteed minimum of 4 hours per day
- Training provided in hoist use, complex personal care, and compression stocking application
- Meaningful, rehabilitation-focused work supporting client independence
- Pathway to permanent part-time employment after 5 months of continuous casual work
- Willing to travel across the Lower North Shore and Northern Beaches
What We're Looking For
- Certificate III or above in Aged Care or Individual Support
- Level 3 Home Care Worker experience preferred
- Minimum 6 months' experience in a similar role
- Current Australian driver's licence
- Reliable vehicle with comprehensive insurance
- Ability to meet all mandatory vaccination requirements (Diphtheria, Tetanus & Pertussis; Hepatitis B; Measles, Mumps & Rubella; Varicella; Tuberculosis; Influenza; and COVID-19)
- Experience supporting high-care or post-hospital clients highly regarded
The Role
As a TACP Home Care Worker, you'll provide post-hospitalisation support to aged care clients across the Lower North Shore and Northern Beaches, including:
- Personal care
- Exercise program support, as directed by Allied Health
- Assistance with compression stockings
- Transporting clients to appointments
- Social support and engagement
- Meal preparation
- Domestic assistance
