About the Role

We have two positions available in this role, and will consider full-time or part-time for the suitable candidates. In this role, based in Wangaratta, you will play a pivotal role in the delivery of the Aged Care Assessment Program (ACAP). The focus of the role is to conduct assessments to work out whether an older person is eligible for government-subsidised aged care.

This role is classified under the Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi Enterprise Agreement 2022 (SACS) at Level 5 ($51-$53.31/hour + superannuation).

Skills & Experience

To thrive in this role, you'll bring:

• Tertiary qualification* in a health-related discipline directly related to health, aged care or related specialist area. E.g. registered nurses, medical officers, occupational therapists, physiotherapists and social workers. 

• Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or part of or eligible to be part of a relevant professional association. 

• Minimum of one year demonstrated experience in Australia or overseas directly delivering services in aged care settings and/or to aged persons (such as geriatric evaluation, rehabilitation, palliative care, community nursing), including to people living with dementia. 

• Current “MAC Learning” training modules and assessments, or willing to complete.
