General Function:

Work closely with consumers to understand their needs and preferences, provide emotional support, assist with personal care, and encourage engagement with family and community. Collaborate with the care team to ensure the delivery of high-quality, personalized home care services.

 

Position Description:

·       Individualised Care: Provide personalised support based on individual care plans, including assistance with daily living activities, hygiene, meals, and mobility.

·       Companionship & Emotional Support: Engage clients in social interaction, activities, and outings, offering empathy and encouragement.

·       Communication & Collaboration: Maintain open communication with clients, families, and the care team, ensuring everyone is informed and aligned.

·       Health & Safety Compliance: Adhere to health and safety regulations, ensuring client safety and comfort at all times.

·       Record Keeping: Accurately document care provided, changes in condition, and other relevant information.

Key Responsibility Areas:

 

  1. Personal Care & Assistance:

    • Assist with bathing, dressing, toileting, etc.

    • Prepare and serve meals

    • Aid with mobility and transfers

  2. Emotional & Social Support:

    • Provide companionship and conversation

    • Encourage social engagement

    • Support hobbies and interests

  3. Healthcare Collaboration:

    • Communicate with healthcare professionals

    • Follow care plans and medical directives

    • Administer medication (if qualified)

  4. Safety & Hygiene:

    • Maintain a safe environment

    • Follow hygiene protocols

    • Report safety concerns promptly

  5. Administrative Duties & Communication:

    • Document services and observations

    • Communicate with care team and family

    • Participate in care planning meetings

 

Essential Knowledge, Skills & Experience

  • Knowledge of personal care techniques

  • Strong communication and interpersonal skills

  • Ability to work with diverse client populations

  • Experience in a home care setting

 

Education, Qualifications and Registrations

  • High School Diploma or equivalent

  • CPR/First Aid Certification

  • Applicable state registration/licensing (if required)

 

Mandatory:

  • National Police Check: A valid and clear criminal history check, specifically including checks for offenses related to aged care or vulnerable individuals.

  • Working with Children Check: If the role involves working with minors.

  • Certificate III in Individual Support (Aged Care): Or equivalent qualification relevant to providing care and support to the elderly.

  • First Aid and CPR Certification: Essential for responding to emergencies.

  • Right to Work in Australia: Proof of citizenship, permanent residency, or a valid work visa.

  • Manual Handling Training: Completion of training in safe lifting and manual handling practices.

  • Driver's License: If the role requires transportation of clients or driving as part of the duties.

  • Infection Control Training: Evidence of training in infection prevention and control.

 

Desirable:

  • Previous Experience in Aged Care or Support Work: Experience working with the elderly or individuals with disabilities would be advantageous.

  • Specialised Training or Certifications: Such as dementia care, palliative care, or other specialized areas of need within aged care.

  • Cultural Competence Training: Understanding and ability to work with diverse cultures, backgrounds, and language groups.

  • Additional Health and Wellness Qualifications: Such as training in nutrition, mental health first aid, or other relevant health areas.

  • Membership in a Relevant Professional Association: Such as the Health and Community Services Union (HACSU) or other professional bodies that reflect ongoing professional development.
