About the role

Ashcare is seeking a compassionate and dedicated Personal Carer / Case Manager to join our community services team. This is a full-time position based in Thornbury, VIC 3071, where you will play a vital role in delivering person-centred support and care coordination to clients with aged care and disability support needs. As a Personal Carer / Case Manager, you will be instrumental in enhancing the quality of life for vulnerable individuals within our community whilst ensuring they receive coordinated, holistic support tailored to their unique circumstances and goals.

Key responsibilities

  1. Provide direct personal care and support to clients, assisting with activities of daily living including personal hygiene, mobility, medication management and meal preparation

  2. Develop and maintain comprehensive care plans in collaboration with clients, their families and other healthcare professionals

  3. Monitor and assess clients' wellbeing, identifying changes in health status and reporting concerns to appropriate team members or external services

  4. Coordinate access to community services, healthcare providers, social programmes and resources to meet clients' individual needs

  5. Maintain accurate and timely case notes and documentation in compliance with organisational policies and regulatory requirements

  6. Provide emotional support and encouragement to clients, promoting their independence, dignity and social participation

  7. Liaise with families, carers, healthcare professionals and community agencies to ensure consistent, quality care delivery

  8. Identify and respond to safeguarding concerns, reporting any suspected abuse or neglect through appropriate channels

  9. Contribute to team meetings and participate in ongoing professional development to maintain and enhance your knowledge and skills


What we're looking for

  1. Current or relevant qualification in aged care, disability support, social work, nursing or related field (Certificate III or above preferred)

  2. Proven experience working in aged care, disability support or community services environments

  3. Strong interpersonal and communication skills, with the ability to build rapport and establish trust with vulnerable clients

  4. Excellent organisational and time management abilities, with capacity to manage multiple clients and competing priorities

  5. Demonstrated understanding of person-centred care principles and commitment to promoting client dignity, rights and independence

  6. Ability to work both independently and collaboratively as part of a multidisciplinary team

  7. Sound judgement and problem-solving skills, particularly in responding to complex care situations

  8. Current Working With Children Check and valid driver's licence (preferred)

  9. First Aid and CPR certification (or willingness to obtain)

  10. Physical fitness and ability to provide hands-on personal care support

  11. Commitment to maintaining confidentiality and upholding professional boundaries
