About the role
Groote Archipelago Regional Council is seeking an experienced and compassionate Aged Care and Disability Services Coordinator to join our Community Services & Development team. This is a full-time position based in Umbakumba, NT 0822, offering the opportunity to make a meaningful difference in the lives of older people and individuals with disabilities across our community.
As the Aged Care and Disability Services Coordinator, you will lead the day-to-day delivery of aged care and disability programs within the community. You will coordinate service delivery, supervise frontline staff, support quality and compliance activities, and work closely with clients, families and service providers to ensure services are safe, culturally appropriate and person-centred. This role plays a key part in maintaining high-quality services, managing operational requirements and contributing to continuous improvement across aged care and disability programs.
Key responsibilities
Coordinate the delivery of aged care and disability services to ensure clients receive high-quality, person-centred support.
Lead, support and develop a team of frontline staff to achieve positive outcomes for clients and the community.
Oversee client assessments, care planning, service reviews and referrals to ensure services remain responsive to individual needs.
Build and maintain effective relationships with clients, families, service providers and community stakeholders.
Monitor service quality, compliance and performance, contributing to continuous improvement initiatives and best-practice service delivery.
Support a safe, inclusive and culturally respectful environment for clients, staff and community members.
Manage operational activities including rostering, reporting, record keeping and service coordination.
Assist with risk management, incident response and quality assurance activities to support organisational and regulatory requirements.
What we're looking for
Relevant qualifications in aged care, disability, community services, health, nursing, social work or a related field, or equivalent demonstrated experience.
Experience working within aged care, disability, health, community services or a related field.
Demonstrated ability to coordinate services, support staff and contribute to positive outcomes for clients and the community.
Strong communication, interpersonal and relationship-building skills.
Sound organisational and problem-solving abilities, with the capacity to manage competing priorities.
A commitment to person-centred service delivery, quality outcomes and continuous improvement.
The ability to work collaboratively with a diverse range of stakeholders, including clients, families, staff and service providers.
Demonstrated cultural awareness and the ability to work respectfully with Aboriginal people and communities.
